Since Frisk gave half of her determination to Chara, Sans is able to see her whilst his and Frisk's friends don't, since he's the only one who can remember the timelines from the reset and been fighting her from the genocide route for like thousands of times.
The left knife is just a normal one from the genocide timeline whilst the one on the right is when Frisk gave half her soul.
The locket that she wears is in her jumper.
